Job Description


The Blended Case Manager provides service coordination to children and adolescents displaying severe social, emotional and/or behavioral disorders and their families. In partnership with the treatment team and any other involved service systems, the case manager develops and monitors a comprehensive service plan for client and family. 

Essential Functions

  • Establishes and maintains linkages with child serving systems such as mental health providers, Children and Youth agencies, drug and alcohol treatment programs, juvenile justice services and the educational system. 
  • Arranges for necessary behavioral health services and communicates relevant information to service providers upon admission or at transfer from one provider to another. 
  • Arranges for aftercare services and communicates with the family and admitting program to ensure that the recommended level of care is accessed in a timely fashion. 
  • Assists in identifying and accessing available and appropriate community resources.  
  • Monitors the effectiveness of services and the family’s satisfaction and compliance with recommend services as part of an ongoing assessment of the need for additional or alternative services. 
  • Meets with client and family at least twice monthly to review progress and revise service coordination plan. 
  • Facilitates interagency team meeting (ITM) in order to coordinate service interventions and understand family’s relationships to community system. 
  • Completes all documentation requirements within determined time-frames; completes all forms, consistent with CGRC corporate compliance procedures. 
  • Completes all training and actively participates in required supervision in accordance with program and agency guidelines. 
  • Meets program expectations regarding productivity. 

Required Education and Experience

  • Bachelor Degree in a counseling, psychology, social work or related behavioral health field;
  • Minimum of one (1) year experience working with children and their families in a mental health, education, child welfare or juvenile justice system (CASSP Systems).  

Preferred Education and Experience

  • Direct experience providing case management.

Work Environment

Services are provided in settings other than CGRC offices including home, school, and other community settings. Role routinely uses standard office equipment such as computers, phones, printers, scanners and photocopiers. Possible exposure to under-resourced home, including homes without heat or air conditioning, and/or those located in geographical areas characterized by high crime rates. 

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. 

While performing the duties of this job, the employee is required to speak and hear sufficiently to communicate effectively by phone or in person at normal volumes. The
employee’s vision must be adequate to read correspondence, computer screen, forms, etc. The position requires good manual dexterity to operate a keyboard, photocopier, telephone, and other office equipment. Employee is regularly required to sit, stand, walk, frequently lift, and/or move up to 10 pounds. 

Position Type/Expected Hours of Work

This is a full-time position working 40 hours per week that consists of meeting a minimum billing requirement, evening hours, possible weekend hours and a rotating on call schedule.  


This position requires up to 75% travel within assigned geographic location.
